Bunions, or hallux valgus, are a common foot deformity that can cause significant pain and discomfort, especially as they progress. In severe cases, patients often face a decision about how best to manage their condition surgically. One approach for severe bunions involves fusing part of the midfoot to correct the deformity, but this can limit mobility and require a longer recovery. The name of this procedure is called a “Lapidus bunionectomy” and it does have its place in appropriate clinical scenarios. Fortunately, not all severe bunions require such drastic measures.
In this post, we’ll explore a case where a patient with a painful and severe bunion found relief without joint fusion, thanks to a distal 1st metatarsal osteotomy with the use of Centrolock orthopedic hardware.
The Challenge with Severe Bunions
In cases of severe bunions, the deformity is typically more pronounced, and the alignment issues can require more significant surgeries like a fusion of the midfoot (Lapidus). For these patients, some surgeons may recommend fusing the bones of the midfoot to provide a long-lasting correction. This procedure, known as a Lapidus fusion, can stabilize the foot but comes with drawbacks like reduced flexibility, a longer recovery, and the potential for complications at the fusion site.
But fusion isn’t always necessary.

A Less Invasive Approach: Distal 1st Metatarsal Osteotomy with Centrolock Intramedullary Fixation
In this case, a patient with a severe, painful bunion sought treatment with the hope of avoiding midfoot fusion. After a thorough evaluation, we decided on a less invasive approach: a distal 1st metatarsal osteotomy using Centrolock intramedullary fixation.
Distal 1st metatarsal osteotomy is a procedure that involves cutting and realigning the first metatarsal bone—the long bone in the foot that connects to the big toe. By making the cut distally (closer to the end of the bone near the toe), we were able to correct the bunion deformity without needing to address the midfoot joints. Because it was a larger deformity, this did require more aggressive correction which can be accomplished with this particular orthopedic hardware set.
Centrolock intramedullary fixation is a type of hardware used to secure the bone after the osteotomy. Unlike traditional screws or plates, Centrolock is placed inside the bone, offering several benefits:

- Stability: The intramedullary design provides secure fixation, reducing the risk of shifting or misalignment after surgery.
- Minimally invasive: A smaller incision can be used compared to more traditional “open” surgery.
- Improved healing: The stable environment created by Centrolock fixation helps the bone heal faster because patients can be more functional faster.
- More aggressive correction: This allows for larger bunions to be corrected without compromising postoperative stability.

The Surgery and Recovery Process
The surgery is performed under regional anesthesia with sedation, and the osteotomy is made at the far end of the first metatarsal. After correcting the alignment, the Centrolock intramedullary device is inserted to stabilize the bone and ensure proper healing.
Post-operatively, the patient is able to bear weight in a surgical boot immediately—significantly sooner than they would have with a midfoot fusion. Physical therapy focused on restoring mobility and strength in the foot, and the patient gradually returned to normal activities without the stiffness often associated with fusion procedures.
Normally, patients can bear weight on the surgical foot immediately post-op. Patients will typically return back into regular sneakers around 4 to 6 weeks post-op. For more active patients typical return back to higher impact activities is usually around 10-12 weeks post-op.

The Results
After healing, the patient is pain-free and walking without difficulty. By avoiding midfoot fusion, the patient retains full range of motion in their foot, allowing them to remain active and comfortable.

While midfoot fusion can be a reliable solution for some cases, it’s not always necessary—even for severe bunions. The distal 1st metatarsal osteotomy with Centrolock intramedullary fixation offers several advantages:

- Quicker recovery: Patients can typically bear weight sooner and get back to their normal activities faster.
