The Pickleball Injury Epidemic

Pickleball has been the fastest-growing sport in America for four consecutive years, with participation surging from 3.46 million players in 2019 to 24.3 million in 2025 — a 171% increase since 2022 alone.

With that explosive growth has come an equally dramatic rise in injuries. Emergency department visits for pickleball injuries rose from approximately 1,313 in 2014 to over 24,461 in 2023. Hospital admissions increased 257% from 2020 to 2022.

Foot and ankle injuries have seen a 6.5-fold increase since 2019, with approximately 70% involving the calf and Achilles complex. UBS estimated $377 million in medical costs from pickleball injuries in 2023.

Common Pickleball Foot & Ankle Injuries

Based on peer-reviewed research from leading orthopedic institutions, these are the most frequent foot and ankle injuries we treat in pickleball players.

39.4%

Achilles Tendon Rupture
The most common pickleball foot and ankle injury. Explosive lunging and push-off during volleys place enormous eccentric load on the Achilles tendon.

16.0%

Gastrocnemius (Calf) Injury
Sudden calf strains from rapid acceleration and deceleration movements on the court.

12.1%

Achilles Tendinopathy
Chronic overuse condition from repetitive impact on hard court surfaces, causing progressive tendon degeneration.

17.7%

Ankle Sprains & Fractures
Lateral shuffling and quick directional changes cause inversion injuries and peroneal tendon strains.

8.0%

Plantar Fasciitis
Hard court surfaces generate repetitive impact forces that drive heel pain and plantar fascia inflammation.

6.8%

Stress Fractures
Overuse injuries from repetitive loading, particularly in players with reduced bone density returning to activity.

Why Pickleball Is Uniquely Hard on Your Feet

Hard court surfaces (concrete, asphalt, sport tiles) generate repetitive impact forces that drive plantar fasciitis, stress fractures, and Achilles tendinopathy
Lateral shuffling and quick directional changes cause ankle sprains and peroneal tendon strains at high rates
The kitchen (non-volley zone) forces abrupt deceleration, placing enormous eccentric load on the Achilles tendon
Explosive lunging during volleys drives Achilles ruptures — the single most common foot/ankle diagnosis
Many players are returning to sport after years of inactivity with baseline deconditioning and reduced bone density
Improper footwear — running shoes lack the lateral support needed for court sports

Protect Yourself on the Court

Proper Court Shoes

Wear court-specific shoes with lateral support — running shoes lack the stability pickleball demands.

Dynamic Warm-Up

Spend 10 minutes warming up your calves, Achilles, and ankles before stepping on the court.

Gradual Progression

Don’t go from sedentary to playing 5 days a week. Build your court time gradually.

Surface Awareness

Hard court surfaces amplify impact. Consider cushioned insoles or custom orthotics.
